Deep under-eye hollows: what to know

Under-eye hollows, or tear troughs, are shaped by the junction between the lower eyelid and cheek. A naturally deep contour may be present early in life. Later, changes in fat compartments, bone support, skin elasticity, or cheek volume can make the depression more noticeable. A shadow from the contour can look like a dark circle, while pigment, veins, irritation, and puffiness are separate contributors.

Sun protection, moisturiser, adequate sleep, and treatment of eyelid irritation can improve the surrounding skin, but they cannot replace lost volume. A clinician should examine the eyelids, cheeks, skin thickness, facial movement, and overall symmetry before recommending treatment.

Arrange prompt care for a hollow that appears suddenly, especially with pain, redness, facial weakness, marked swelling, or vision symptoms. After any under-eye procedure, severe pain, skin colour change, increasing swelling, or blurred or reduced vision is urgent. A stable hollow without warning signs can be assessed at a routine appointment.
