Decidua immunohistochemistry: what the result means

The decidua is the uterine lining after it has changed in response to pregnancy-related hormones. Under a microscope, decidual cells can sometimes resemble other large cells, including trophoblastic tissue or a tumour. Immunohistochemistry adds labelled antibodies to the tissue sample; the resulting staining pattern helps the pathologist clarify which cell types are present. A panel of stains may be more informative than any single marker.

A report mentioning decidua is not, by itself, a cancer diagnosis. It also does not by itself confirm that a pregnancy is located inside the uterus, because decidual change can occur without chorionic villi in the sample. The pathologist interprets the stains alongside the tissue appearance, the type of specimen, imaging, pregnancy test trends, symptoms, and clinical history.

Review the final pathology impression rather than trying to interpret a positive or negative stain in isolation. Ask whether chorionic villi, trophoblastic cells, atypical cells, or another specific finding was seen and whether the sample was adequate. Your clinician may compare the pathology with ultrasound findings and blood tests, or request additional stains or tissue review if the results do not fit the clinical picture. Follow the arranged plan even if your symptoms have improved.

Seek urgent medical assessment for heavy vaginal bleeding, severe or worsening abdominal or pelvic pain, shoulder-tip pain, fainting, marked dizziness, shortness of breath, or weakness. These symptoms require prompt evaluation, especially when the pregnancy location is uncertain or after a uterine procedure. Contact the treating team promptly as well for fever, foul-smelling discharge, or increasing pain after tissue sampling.

Useful questions include: What tissue did the stains identify? Were chorionic villi or trophoblastic cells present? Does the result match my ultrasound and blood tests? Is any atypical or malignant tissue suspected? Do I need repeat testing, further sampling, or review by a specialist pathologist?
