De Quervain release surgery: what to know

De Quervain's tenosynovitis affects tendons that move the thumb as they pass through a narrow compartment near the wrist. Swelling and friction can cause pain on the thumb side of the wrist, especially with gripping, lifting, or thumb movement. Release surgery involves a small incision and opening of the compartment so the tendons have more room to glide. The surgeon takes care around nearby nerves and blood vessels. The procedure may be offered when symptoms remain limiting despite appropriate non-surgical care, or when the clinical findings make release a reasonable option.

Before surgery, discuss medicines that affect bleeding, allergies, work demands, and whether you will need help with dressing, cooking, or childcare while the hand is protected. Afterward, keep the dressing and incision as instructed, elevate the hand when swollen, and move the fingers unless told otherwise. Avoid forceful gripping and heavy lifting until the surgeon allows them. Scar sensitivity, swelling, and altered thumb comfort can take time to settle; hand exercises or therapy may be advised if movement remains restricted.

Contact the surgical team promptly if the dressing becomes soaked, the incision opens, pain is worsening despite the prescribed plan, or redness and warmth spread around the wound. Seek urgent help for severe swelling with numbness, a blue or cold finger, or inability to move the thumb or fingers. Fever or pus-like drainage also warrants prompt review. Do not wait for a routine appointment when circulation or sensation changes suddenly.
