Dark underarms and neck: what to know

The underarms are exposed to rubbing, sweat, shaving, and deodorant irritation. The neck can also react to friction, hair products, jewellery, or inflammation. Colour may remain after the original irritation has settled; this is called post-inflammatory hyperpigmentation. Intertrigo may affect warm, moist folds and usually brings surface symptoms as well as colour change.

When the neck and underarms become darker with a thicker, velvety feel, the pattern is different from a flat mark after irritation. A clinician will consider the texture, speed of onset, medicines, other affected folds, and general health rather than diagnosing from colour alone.

Cleanse gently, dry skin folds carefully, and reduce rubbing with breathable clothing. Use fragrance-free products if possible and stop anything that burns. Avoid abrasive scrubs and unapproved lightening mixtures, which can inflame both the underarms and neck.

If the skin is calm but pigment remains, a dermatologist can discuss chemical peel, laser toning, laser hair removal for shaving-related irritation, or hydroquinone treatment where appropriate. Treatment should follow an examination, especially when the neck is involved or the skin feels thick.

Arrange a review if both areas darken without an obvious irritant, if the change spreads, or if the skin becomes thick or velvety. Seek prompt care for pain, warmth, swelling, pus, fever, marked scaling, or a rapidly evolving rash. A photograph from when the change began can help show progression.
