Dark spots near underarms: causes and next steps

Flat colour after shaving or a rash can be post-inflammatory hyperpigmentation. A darker, velvety, thicker texture in the underarms or neck deserves a clinician’s review because it can occur alongside insulin resistance and other metabolic concerns. It is not possible to identify the cause from colour alone. Redness, odour, scale, tenderness, or itch may indicate an active inflammatory or infectious problem. Note when it began and whether other folds are affected.

Wash gently, dry the fold carefully, and choose loose breathable clothing. Pause a deodorant or hair-removal product if it burns, and avoid vigorous scrubbing. Moisturise irritated skin with a bland product. If the skin is thickened or the change is widespread, arrange a medical review rather than treating only the colour. Arrange assessment for sudden or progressive darkening, thick velvety skin, persistent itch, scale, pain, cracks, discharge, or a lump. A clinician may also check related skin folds and discuss whether blood glucose evaluation is appropriate.
