Dark spots and blemishes: understanding the difference

Short answer
Dark spots and blemishes are not the same: a blemish is active inflammation, while a dark spot is often pigment left after it settles.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Active blemish
- May be red, swollen, tender, or fluid-filled
- Dark mark
- Often flat pigment after inflammation
- First step
- Avoid picking and protect exposed skin
Spot versus mark
A blemish may be red, swollen, tender, or contain a visible plug or fluid. A post-blemish mark is usually flat and brown, grey, or darker than nearby skin, without the same active swelling. Picking can prolong inflammation and increase the chance of pigmentation or scarring. Similar-looking patches can occur with [Melasma](/conditions/melasma/), [Sun Spots](/conditions/sun-spots/), [Tinea Versicolor](/conditions/tinea-versicolor/), or other [Dark Patches On Skin](/symptoms/dark-patches-on-skin/), so appearance alone cannot confirm the cause.
A calmer routine
Use a mild cleanser, moisturiser, and daily broad-spectrum sunscreen on exposed skin. Avoid squeezing active blemishes and avoid applying abrasive scrubs or several strong products at once. Focus first on controlling new breakouts; otherwise fresh inflammation can keep adding marks. If pigmentation remains bothersome, a clinician may consider [Chemical Peel](/treatments/aesthetic-procedures/chemical-peel/), [Skin Brightening Treatment](/treatments/aesthetic-procedures/skin-brightening-treatment/), or [Picosecond Laser Treatment](/treatments/laser-and-energy/picosecond-laser-treatment/), depending on skin sensitivity and the diagnosis.
When to seek care
Seek advice for deep painful blemishes, repeated pus-filled lesions, spreading redness, or scarring. Have any dark spot assessed if it is changing, bleeding, raised, persistently itchy, or unlike the other marks. Professional help is also useful when breakouts or pigmentation are affecting sleep, confidence, or daily routines.
Questions for your doctor
Which spots are active blemishes and which are leftover pigment? What routine can address both without irritation? Should I prioritise acne treatment, pigment care, or an examination of any unusual mark?

Questions people ask
Why do blemishes leave dark spots?
Inflammation can stimulate extra pigment as the skin heals, especially after squeezing or repeated irritation.
Can a dark mark still be an active spot?
A flat mark without swelling is often leftover pigment, but a clinician can check an itchy, raised, painful, or changing area.
