Dark Elbows and Underarms: What to Know

Elbows are exposed to pressure and rubbing, while underarms experience moisture, shaving, deodorant contact, and skin-on-skin friction. Dryness or repeated irritation can leave darker pigment after inflammation fades; this is known as Post-Inflammatory Hyperpigmentation. Red, moist, tender, or cracked folds may fit Intertrigo. Thickened skin, itch, scale, or a rash in other body sites suggests that pigment may be only one part of the problem.

The same colour change can have different causes in each area. Note whether the skin is smooth, rough, raised, itchy, sore, or damp before choosing a cosmetic treatment.

Wash gently, dry skin folds carefully, and moisturise rough elbows with a plain fragrance-free product. Reduce leaning on the elbows and choose breathable clothing. Pause fragranced deodorant, aggressive scrubbing, and hair removal if the underarms sting. Do not use hydroquinone or strong acids without clinician supervision, especially in folds where irritation can worsen pigment.

Arrange an assessment if darkening is spreading, the skin becomes thick or velvety, or there is persistent itch, scale, pain, cracking, odour, or moisture. Seek prompt care for rapidly spreading redness, warmth, pus, fever, or severe tenderness. New widespread darkening should be reviewed rather than treated only as a cosmetic concern.

Ask whether friction, Intertrigo, inflammation, or another skin change is responsible; which products and hair-removal methods are suitable; and whether pigment treatment should wait until the skin barrier is calm.
