Dark Brown Bleeding During Pregnancy

Dark brown bleeding during pregnancy often reflects older blood leaving the vagina, but any bleeding should be reported to a maternity clinician because the cause can range from cervical irritation to a pregnancy complication. The colour alone cannot show whether the bleeding is harmless. Note when it began, whether it is spotting or flowing, and whether you have pain, cramps, fever, dizziness, or a change in vaginal fluid.

Blood that has taken longer to leave the uterus or vagina can look brown rather than bright red. The cervix may bleed more easily during pregnancy, including after sex or an internal examination. Infection, a small collection of blood, or changes involving the placenta can also cause bleeding. In early pregnancy, bleeding may occur with a normally developing pregnancy, but it can also accompany miscarriage or an ectopic pregnancy. Later in pregnancy, the placenta, cervix, or beginning of labour may be involved.

A clinician may ask about the amount and colour of blood, examine the cervix, check the pregnancy, and arrange an ultrasound or blood tests when appropriate. A pregnancy-related bleed is not the same as ordinary Bleeding Between Periods, so use the maternity contact provided by your hospital or clinic for tailored advice.

Contact your obstetrician, midwife, maternity unit, or pregnancy clinic and describe the bleeding. Use a sanitary pad to observe the amount; avoid tampons while bleeding. Rest if you feel unwell, keep hydrated, and avoid sex or anything inserted into the vagina until a clinician has advised you. Do not self-diagnose from the colour or stop prescribed medicines without medical advice. Tell the clinician how far along the pregnancy is, your blood group if known, whether you have had a recent examination or sex, and whether fetal movement has changed.

Seek emergency care now for heavy or rapidly increasing bleeding, severe or one-sided abdominal pain, shoulder-tip pain, fainting, marked dizziness, weakness, or collapse. These symptoms can signal significant blood loss or an ectopic pregnancy. Seek urgent maternity advice as well for fever, foul-smelling discharge, regular painful contractions, leaking fluid, or reduced fetal movement once you normally feel movement. If you cannot reach your maternity team, use local emergency services.

Even light brown spotting deserves prompt advice if it recurs, lasts, follows sex or an examination, or occurs with cramps. Keep any appointment arranged for assessment, even if the bleeding stops.

Ask what may be causing the bleeding at this stage of pregnancy, whether you need an examination or ultrasound, which symptoms should send you to emergency care, and whether you should change activity or avoid sex temporarily. If you are Rh negative or do not know your blood group, ask whether you need a blood-group assessment or treatment.
