What should you know about cystocele grades?

Cystocele grades describe how far the front wall of the vagina and bladder have descended, from mild support changes to tissue reaching or extending beyond the vaginal opening. A grade is one part of the assessment; symptoms, bladder emptying, examination findings and your daily needs also guide care.

Clinicians may use a numbered grading or staging system. Lower grades generally describe a bladder that remains well supported inside the pelvis, although pressure, urinary leakage or a feeling of incomplete emptying can still occur. Middle grades indicate that the bulge has moved closer to the vaginal opening. A higher grade means the front vaginal wall reaches the opening or protrudes beyond it, especially with standing, coughing or straining.

The number does not measure discomfort on its own. Some people with a visible bulge have few symptoms, while others with a smaller descent notice pelvic pressure, urinary changes or difficulty during activity. A vaginal bulge is a common presentation of cystocele, but a bulge can have other causes, so an examination is needed to identify what is happening.

Arrange an assessment if you notice pressure, a vaginal bulge, urinary leakage, repeated difficulty emptying your bladder or symptoms that interfere with walking, exercise or sex. The clinician may ask when the bulge appears, examine you while relaxed and bearing down, and check whether your bladder is emptying properly.

Treatment depends on symptoms, grade, general health and preferences. Options can include observation with follow-up, pelvic-floor rehabilitation, managing constipation and avoiding repeated straining. A fitted vaginal pessary can support the vaginal wall. Pelvic floor repair surgery may be discussed when prolapse symptoms remain troublesome or when conservative measures are not suitable.

Seek prompt medical advice if you cannot pass urine, have increasing difficulty emptying your bladder, or the exposed tissue becomes very sore, swollen, bleeding or difficult to keep clean. Urgent help is also appropriate for severe pelvic pain, fever or feeling acutely unwell. These symptoms need assessment regardless of the cystocele grade.
