Cutaneous diphtheria: what to know

How the structures involved in Diphtheria differ from normal

Short answer

Cutaneous diphtheria is a bacterial infection of the skin that can affect an ulcer or wound and requires prompt clinical assessment and infection control.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What it may look like

The infection may cause a slow-healing sore, ulcer, crust or greyish coating, with redness, swelling or discharge around it. Some people have few symptoms beyond the wound. The bacteria can spread through contact with infected skin or contaminated dressings. Cutaneous infection is different from throat diphtheria, although throat symptoms such as a sore throat need urgent attention because diphtheria can affect more than the skin.

What to do

Contact a clinician promptly about a suspicious or non-healing wound, especially after travel or close contact with someone who has a similar infection. Cover the area, wash hands carefully, avoid sharing towels, and dispose of dressings safely according to medical advice. Do not pick the coating or use leftover medicine. A clinician may take a swab and arrange antibiotic treatment, while public-health advice may be needed for close contacts. Check whether childhood immunisation and adult booster protection are up to date.

When to seek urgent care

Seek urgent help for trouble breathing or swallowing, a sore throat with marked weakness, facial or neck swelling, confusion, fainting, rapidly spreading redness or fever. Keep the wound covered and mention possible diphtheria when arranging care so staff can advise on precautions.

Questions for your doctor

Ask whether the wound needs a swab, which treatment is appropriate, how long contact precautions should continue, whether household contacts need assessment, and whether your immunisation record needs review.

Questions people ask

Can cutaneous diphtheria spread to other people?

Yes. Contact with infected skin or contaminated dressings can spread bacteria, so cover the wound and follow infection-control advice.

Is a sore throat part of every skin infection?

No. A sore throat may signal a different or more extensive form of diphtheria and needs urgent assessment.
