Cupping for lymphedema: what to know

The suction can cause bruising, broken skin or inflammation in an area where lymph drainage is already reduced. Wet cupping adds punctures and therefore another route for infection. Do not apply cups over an affected limb or body area unless your lymphedema clinician has specifically assessed the plan.

Lymphedema is swelling caused by impaired movement of lymph fluid. It can follow lymph-node surgery, mastectomy or radiotherapy, although it can also have other causes. Skin in a swollen area may feel tight and can be more vulnerable to cracking, irritation and infection.

Cupping creates negative pressure rather than the carefully directed, gentle techniques used in specialist lymphatic care. Temporary marks do not show that trapped fluid or toxins have been removed.

Arrange an assessment with a lymphedema therapist. Management may include fitted compression, movement, skin care and specialised massage when suitable. Keep the skin clean and moisturised, protect it from cuts and burns, and follow the garment instructions given for your swelling.

If you already had cupping, remove the cups and inspect the area without rubbing it. Note new redness, blisters, bleeding, warmth, pain or extra swelling, then tell your treating team what technique was used and whether the skin was pierced.

Seek urgent medical assessment for rapidly spreading redness, increasing warmth, marked tenderness, fever, chills or feeling suddenly unwell. These can accompany a skin infection that needs prompt treatment. Sudden major swelling, chest pain or difficulty breathing also needs emergency care.

Contact your lymphedema service soon if swelling is increasing, a garment no longer fits, fluid leaks through the skin, or any cupping wound is slow to close.

Ask which type of compression suits the affected area, how to recognise infection, whether specialist lymphatic massage is appropriate, and which exercises are safe after surgery or radiotherapy. Bring details of any complementary therapy you are considering so the team can check its effect on your skin and swelling.
