What densities mean on a CT scan

Air appears very dark, while dense structures such as bone look bright. Fat, fluid, soft tissue, blood and calcification have different appearances, but density alone rarely identifies exactly what a finding is.

Radiologists assess more than shade. They consider a finding's location, shape, borders, internal pattern and behaviour after contrast, then compare it with surrounding tissue and any earlier scans. Words such as low density, high density or mixed density describe appearance; they are not diagnoses by themselves.

An imaging contrast agent can increase the brightness of blood vessels and tissues with a blood supply. Comparing images before and after contrast may help separate fluid from solid tissue or show enhancement. Scanner settings also change how the same data are displayed, allowing the reader to examine lungs, soft tissues or bones more clearly.

Focus on the report's impression, where the radiologist summarises the meaningful findings and any suggested next step. A density term in the descriptive portion may simply record what was seen. Compare the wording with prior imaging when available, because stability or change can influence interpretation.

Ask the referring clinician what structure the term describes, whether it has specific or uncertain features, and whether follow-up imaging is recommended. If another examination is suggested, clarify whether its purpose is to characterise the tissue, check for change or guide a procedure.

A density phrase alone does not determine urgency. Seek prompt care for the symptom that led to scanning if it becomes severe or rapidly worsens, especially new breathing difficulty, sudden weakness, fainting or uncontrolled bleeding. After contrast, breathing trouble or swelling of the face or throat requires emergency help.

Useful questions include whether the density is new, whether it changed after contrast, whether its borders or location are reassuring or uncertain, and whether earlier scans alter the interpretation. Also ask who will arrange any recommended follow-up and what symptoms should prompt earlier contact.
