CSF liquid biopsy: what patients should know

The laboratory may look for fragments of tumour DNA or other markers. The result is interpreted alongside scans, symptoms, examination findings and, when available, tissue testing; it does not automatically replace a tissue biopsy.

CSF is the clear fluid surrounding the brain and spinal cord. A sample is commonly obtained by lumbar puncture, in which a clinician places a needle into the lower back after numbing the skin. Collection may not be suitable when pressure around the brain is raised or when bleeding and infection risks need attention, so the team checks safety first.

Finding a relevant molecular change may support diagnosis, help match the tumour profile, or allow changes to be followed over time. A result that finds no tumour material cannot by itself exclude cancer because the sample may contain too little material or the test may not cover every possible alteration.

Before sampling, give the team a current list of medicines and mention blood-thinning treatment, bleeding problems, allergies, pregnancy, fever, or a skin infection near the lower back. Follow the clinic's instructions about food, drink and whether any medicine needs adjustment; do not stop prescribed treatment on your own.

Afterward, follow the discharge advice and ask when the molecular report will be ready. At the results visit, clarify which marker was found, whether it changes the working diagnosis or treatment plan, and whether comparison with tumour tissue or a later CSF sample is needed.

Contact the clinical team promptly for a severe or persistent headache, repeated vomiting, fever, increasing back pain, redness or fluid leaking from the puncture site, new weakness or numbness, confusion, or difficulty passing urine. Seek emergency care for a seizure, loss of consciousness, sudden major weakness, or rapidly worsening neurological symptoms.

Ask what the laboratory is testing for, how the answer could alter care, and what a negative or uncertain result would mean in your case. It is also useful to ask who will perform the lumbar puncture, what safety checks are planned, whether another diagnostic method is still required, and whom to contact after leaving the clinic.
