Croup and sore throat: what to know

Croup is an illness affecting the upper airway around the voice box. Swelling there can cause a barking cough, hoarseness and a harsh sound when breathing in; irritation may also feel like a sore throat. Children may describe pain poorly, so changes in drinking, speech or crying can provide useful clues.

Sore throat can accompany several illnesses, including a common cold, influenza or COVID-19. Tonsillitis and strep throat can cause more prominent throat pain, fever or difficulty swallowing, but symptoms overlap and cannot confirm a diagnosis by themselves. Acid reflux or allergic rhinitis can irritate the throat without causing typical croup. Keep the child calm and upright, especially during coughing episodes. Offer cool or room-temperature fluids in small, frequent amounts if swallowing is comfortable. Soft foods may be easier than rough or spicy foods. Follow the label or a clinician’s instructions for age-appropriate pain relief, and check with a pharmacist if the child takes other medicines or has a medical condition.

Do not force fluids when breathing is hard, and do not use steam inhalation. Avoid sharing cups and utensils while an infectious illness is possible. Note when the cough and throat pain started, whether the voice is hoarse, whether breathing is noisy while resting, and how much the child is drinking. A clinician can decide whether an examination or targeted test is needed; leftover antibiotics should not be used.

Call emergency services if the child is struggling for breath, has noisy breathing while calm and resting, cannot swallow saliva, is drooling with distress, develops blue or grey lips, or becomes difficult to wake. Keep them upright and minimise crying while help is arranged.

Seek same-day medical advice for worsening throat pain, inability to drink, repeated vomiting, signs of dehydration, a muffled voice, or symptoms that are not settling. Urgent assessment is also needed if the child looks seriously unwell or has a stiff neck. A sore throat alone does not establish croup or strep throat; urgency depends on breathing, swallowing, hydration and overall appearance. Ask: Is the throat pain part of croup, or does it suggest tonsillitis or strep throat? Does my child need a throat examination or test? Which pain-relief option and dose are appropriate? What breathing or swallowing changes should prompt emergency help? Tell the clinician whether there is drooling, a hoarse voice, fever, rash, reduced drinking, vomiting, or noisy breathing at rest.
