What croup and laryngitis mean

Croup is inflammation around the upper airway, commonly after a viral infection. It is seen mainly in babies and young children because their airways are narrower. A harsh, barking cough, a hoarse cry, and a high-pitched or raspy sound when breathing in can occur, often becoming more noticeable at night.

Laryngitis is inflammation of the larynx, or voice box. It can follow a cold, voice strain, irritation from smoke or other substances, or acid reflux. Hoarseness, a weak voice, throat irritation, and a dry cough are typical. Laryngitis can affect people of any age, and croup-like symptoms in an adult need a different assessment.

What you can do

Keep a child with possible croup calm and upright, because crying can make noisy breathing worse. Offer frequent sips of fluid and watch how comfortably they breathe. Avoid steam inhalation, which can burn the skin and does not reliably open the airway. Do not give cough medicines to a young child unless a clinician or pharmacist advises it.

For laryngitis, rest the voice, drink fluids, and avoid whispering because it can strain the voice box. Avoid smoking and smoky surroundings. If reflux seems to trigger hoarseness, a GP can discuss symptoms such as heartburn, sour fluid in the throat, or a cough after meals. A cold or sore throat may accompany laryngitis, but persistent voice change deserves assessment.

When to seek care

Seek emergency help if a child is struggling to breathe, has blue or grey lips, becomes unusually sleepy, cannot swallow saliva, or makes noisy breathing while resting. Keep them upright and calm while help is arranged. Do not examine the throat repeatedly if breathing is difficult.

Arrange prompt medical advice if symptoms are worsening, breathing remains noisy at rest, the child is drinking much less, or there are signs of dehydration such as very few wet nappies or passing little urine. A GP can assess the airway and decide whether treatment is needed. Adults should arrange an appointment for hoarseness that does not settle, keeps returning, or occurs with difficulty swallowing, neck swelling, coughing blood, or unexplained weight loss.
