Cribriform prostate cancer: understanding the pathology

Under the microscope, cribriform cancer forms rounded or irregular sheets of cells containing multiple openings. It is a feature within prostate adenocarcinoma rather than a symptom or a separate organ cancer. The pathology report may distinguish invasive cribriform growth from intraductal carcinoma, which is related but recorded separately.

Clinicians combine this architecture with the Grade Group, tumour extent, prostate-specific antigen pattern, examination and imaging. A second pathology opinion may be useful when the wording is uncertain or when the finding could change a major treatment decision.

Meet a urologist and radiation oncologist to review whether disease appears confined to the prostate or has spread. Depending on the full assessment, choices can include prostate removal surgery, external radiotherapy or brachytherapy, sometimes combined with medicines that suppress androgen signalling. More extensive disease may require systemic treatment, with targeted radioisotope therapy or bone-strengthening therapy considered in selected settings.

Ask how urinary control, sexual function, bowel effects and recovery differ between suitable options. Your personal priorities and other health conditions belong in the decision alongside cancer features.

Seek urgent care if you cannot pass urine, develop new leg weakness or numbness, lose bladder or bowel control, or have severe unexplained back pain with neurological changes. Contact the treating team promptly for visible blood in urine, worsening difficulty passing urine, new persistent bone pain or treatment side effects that prevent drinking. These symptoms require assessment because of their potential consequences; none identifies the cribriform pattern on its own.

Ask whether the report shows invasive cribriform cancer, intraductal carcinoma or both, and how much tissue contains the finding. Confirm your Grade Group and stage, whether additional imaging is appropriate, which treatment paths remain reasonable and how follow-up will detect a response or recurrence.
