What may be happening
The cremaster muscle lies in the spermatic cord region and responds to temperature, touch, and reflex signals. A sudden movement, forceful exercise, coughing, or direct impact may irritate nearby muscle or connective tissue. Pain can feel like a pull in the groin, inner thigh, or scrotum and may worsen with movement or muscle tightening.
Groin pain is not specific to a muscle injury. Hernia, testicular torsion, infection, kidney stones, and other causes can feel similar. A clinician may examine the abdomen, groin, and scrotum and decide whether imaging or another test is needed.
What to do initially
Stop the activity that triggered the pain and use comfortable support for the scrotum. Cold applied over a cloth for short periods may ease early discomfort. Avoid heavy lifting, sprinting, and stretching into pain. Once urgent causes have been excluded and symptoms settle, gentle movement can be resumed gradually; a clinician or physiotherapist can guide a return to exercise.
Use pain medicine only if it is suitable for you and follow the label or professional advice. Do not massage a painful or swollen testicle, and do not assume persistent scrotal pain is a simple strain.
When to seek urgent care
Seek emergency assessment for sudden severe testicular pain, a testicle that appears higher or differently positioned, rapid swelling, nausea or vomiting with scrotal pain, fever, a groin bulge that cannot be pushed back, or pain after a significant injury. Prompt review is also needed for blood in urine or pain that is worsening rather than settling.
