What does yellow crust in cradle cap mean?

Cradle cap can look yellow because scalp oil and shed cells dry together into a soft or firmly attached scale. The surrounding skin may be mildly pink. Yellow scale alone does not show that the scalp is infected, particularly when there is no tenderness, swelling, spreading redness, or fluid. Similar-looking crust can have other causes, so the whole pattern matters.

Soften the area with a small amount of baby-safe emollient, then wash with mild shampoo and lukewarm water. Use a soft brush only for flakes that have loosened. Never scrape, peel, or use fingernails on attached crust. Keep products away from the eyes and rinse thoroughly. If the crust keeps returning, covers much of the scalp, or comes with irritation, a clinician can advise whether topical antifungal treatment is appropriate.

Contact a clinician if yellow crust is wet, foul-smelling, bloody, painful, swollen, or surrounded by increasing redness. Seek prompt care if your baby has fever, appears unusually sleepy, feeds poorly, or seems distressed. A review is also useful when gentle care has not helped or when the rash extends onto other body areas.
