Cradle cap scabs: what they may mean

Typical cradle cap is flaky or greasy scale on the scalp. A firm brown or yellow crust may form when scale dries, but a scab can also follow scratching or broken skin. The appearance alone cannot confirm the cause. Look for whether the area is moist, tender, swollen, or expanding beyond the original patch.

Keep fingernails short and avoid picking or peeling the crust. Wash gently with mild baby shampoo, rinse fully, and pat dry rather than rubbing. A plain emollient may soften attached scale, but do not apply it to a wet, oozing, or visibly infected area without clinical advice. If the scalp stays inflamed, a clinician can decide whether emollient therapy or topical antifungal treatment is appropriate.

Seek medical advice for pus, repeated bleeding, warmth, swelling, increasing tenderness, or a bad smell. Prompt assessment is needed if the baby has fever, seems unusually sleepy, feeds poorly, or the rash spreads quickly. These findings are not explained by simple dry scale alone and should be assessed rather than treated by scraping.
