What to know about cradle cap on the neck

A neck rash may occur below the hairline or within one or more folds. Drool, milk, sweat, and rubbing can keep the fold damp and make the skin sore. Cradle cap can involve nearby skin, yet a bright-red, tender, moist rash or small separate spots may need a clinician’s assessment for another cause. The direction of spread and the condition of the skin matter more than the name used at home.

Clean the fold with lukewarm water or a mild cleanser, then pat it completely dry without rubbing. Change damp bibs and clothing promptly, and allow air to reach the fold during supervised care. Do not peel scale, use talcum powder, or apply fragranced products. An emollient may be useful for dry surrounding skin, but a clinician should advise whether it suits a moist fold. If the rash continues, ask whether topical antifungal treatment or another option is appropriate rather than treating by guesswork.

Arrange an appointment if the area remains wet, cracked, painful, or inflamed despite gentle cleaning and drying. Contact a clinician promptly for pus, spreading redness, foul smell, fever, or a baby who is unusually unsettled. A rash that reaches the chest or face, or keeps returning, also deserves an examination because folds can hide irritation or infection.
