Constant Neck and Shoulder Stiffness: What to Know

Short answer
Constant neck and shoulder stiffness deserves assessment when it persists, restricts movement, or comes with weakness, numbness, fever, or recent injury.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

- Neck motion, shoulder motion, strength, sensation, and reflexes
The short answer
Muscle tension, sustained posture, reduced activity, joint irritation, and shoulder disorders can all limit comfortable motion. The pattern matters: notice whether the restriction starts in the neck or shoulder, whether one side is worse, and which movements provoke it.
What the pattern may mean
Stiffness describes restricted or effortful movement, which is different from pain alone. A stiff shoulder can make reaching overhead or behind the back difficult, while neck restriction may show up when turning to check traffic. Frozen shoulder commonly presents with progressive pain and marked loss of both active and assisted shoulder movement. Shoulder osteoarthritis can present with stiffness, aching, and grinding, whereas a rotator cuff tear more often presents with pain or weakness and may also reduce motion.
A clinician can compare neck and shoulder movement, strength, sensation, and reflexes. This helps distinguish protective muscle guarding from a joint problem or irritation of a nerve traveling from the neck toward the arm.
What you can do now
Keep the area moving within a comfortable range instead of forcing a stretch. Change position regularly, support the forearms during desk work, and place frequently used items where reaching does not strain the shoulder. A warm shower or wrapped heat pack may ease muscle guarding if heat is safe for your skin and sensation.
Arrange a routine appointment if the stiffness remains constant, keeps returning, disturbs sleep, or limits dressing, driving, or work. Bring a brief record of the onset, aggravating movements, previous injuries, and any arm symptoms.
When to seek urgent care
Seek urgent medical care after a significant fall or collision, or if stiffness accompanies new arm weakness, spreading numbness, poor balance, loss of bladder or bowel control, fever with severe neck pain, a sudden intense headache, chest pressure, breathlessness, sweating, or pain spreading to the jaw or arm. These features need prompt assessment regardless of how the stiffness began.
Useful questions for your clinician
Ask whether the movement loss appears to originate in the neck, shoulder joint, or surrounding muscles; which movements are safe to practise; whether weakness or altered sensation changes the plan; and what change should trigger reassessment.

Questions people ask
Can desk posture contribute to neck and shoulder stiffness?
Yes. Holding the head and arms in one position can fatigue muscles and reduce comfortable movement. Regular position changes and supported forearms may help, but persistent restriction still merits assessment.
Should I stretch a constantly stiff shoulder?
Gentle movement within a tolerable range may be reasonable, but forceful stretching can aggravate an irritable joint or injured tissue. Stop if it causes sharp pain, tingling, or increasing weakness.
