Concealed placental abruption symptoms

Placental abruption happens when the placenta separates from the womb wall before birth. In a concealed abruption, blood can remain trapped behind the placenta, so the amount of vaginal bleeding does not show how serious the problem may be. Pain may be constant or may come with tightening, and the abdomen can feel unusually firm or sore. Fetal movements may become less noticeable, although movement changes are not the only warning sign.

This is an obstetric emergency because separation can affect oxygen and blood flow to the baby and can cause illness in the pregnant person. Assessment usually includes checking vital signs, the abdomen, contractions and the baby's heart rate. Ultrasound may be used, but a scan cannot always exclude an abruption.

If you are pregnant and have unexplained abdominal or back pain, a tender or hard-feeling womb, repeated contractions, or reduced fetal movement, contact your maternity unit immediately. Do not wait for visible bleeding. Follow the clinician's advice about coming to hospital, and avoid driving yourself if you feel faint, weak or severely unwell. At hospital, clinicians may monitor you and the baby, give fluids or blood products when needed, and decide whether birth is safest. Seek emergency maternity care now for severe or worsening pain, a rigid or very tender abdomen, faintness, dizziness, heavy bleeding, contractions that are close together, or reduced or absent fetal movement. Call emergency services if you collapse, have trouble breathing, or cannot travel safely. The absence of vaginal bleeding does not make these symptoms safe to watch at home.
