Compression of the cervical spinal cord

The pressure can disrupt signals between the brain and the body. New weakness, difficulty walking, loss of bladder or bowel control, or rapidly worsening neck pain should be treated as urgent warning signs.

The cervical spinal cord carries nerve signals for the arms, legs, bladder, bowel, and other functions. Compression may cause neck pain, pain travelling into an arm, hand clumsiness, tingling, numbness, limb weakness, unsteady walking, or changes in bladder or bowel control.

Possible causes include narrowing around the cord, injury, infection, bleeding, or a tumour involving the spine. In oncology, a cancer deposit or a blood cancer such as multiple myeloma can weaken vertebrae or form tissue that presses on the cord. Symptoms cannot identify the cause by themselves.

Keep the neck in a comfortable, supported position and avoid forceful manipulation while seeking assessment. A clinician will check strength, sensation, reflexes, walking, and bladder or bowel symptoms. Imaging is used to locate the pressure and guide care.

Treatment depends on the cause, the degree of compression, overall health, and the goals of care. Medicines may reduce swelling around the cord. Radiotherapy may shrink a sensitive tumour, while surgery may stabilise the spine or remove pressure. These options can be used separately or in combination.

Contact emergency services or the oncology emergency line immediately for new or worsening arm or leg weakness, trouble standing or walking, numbness around the buttocks or genitals, inability to pass urine, loss of bladder or bowel control, or severe neck pain with neurological symptoms. Do not wait for a routine appointment or drive yourself if movement is unsafe.

Ask what is causing the compression, whether the spine is stable, which movements are safe, and what treatment needs to begin now. You can also ask how pain will be controlled, how strength and walking will be monitored, and whether rehabilitation or mobility support is appropriate.
