The practical answer
A complete tear does not automatically mean an operation. Some isolated tears can be managed with a structured rehabilitation program and an appropriate brace, while combined ligament damage, persistent instability or demanding activity goals may shift the discussion toward reconstruction.
How a complete PCL tear affects the knee
The posterior cruciate ligament runs inside the knee and limits backward movement of the shin relative to the thigh. It can tear when a bent knee receives a force to the upper shin, during a fall onto the knee or in a high-energy collision.
Swelling, pain behind or throughout the knee, difficulty descending slopes or stairs, and a sense that the knee is unreliable can accompany a PCL injury. Clinicians examine backward movement of the shin and check the other ligaments, cartilage, menisci, nerves and circulation. Imaging helps define the tear and associated damage.
Early protection and rehabilitation
After a recent injury, reduce activities that provoke instability, use cold therapy with a cloth barrier for swelling, and follow guidance on weight-bearing. A brace designed for PCL injuries may support the shin in a better position; a generic sleeve does not provide the same control.
Rehabilitation commonly prioritises swelling control, knee motion, quadriceps strength, hip and trunk control, balance and a gradual return to task-specific movement. Avoid unsupported deep knee bending or hamstring loading early if your clinician says these movements could pull the shin backward.
When the knee needs prompt assessment
Seek urgent care after knee trauma if the foot becomes cold, pale, numb or weak; the knee looks deformed; pain or swelling escalates rapidly; or you cannot safely bear weight. High-energy injuries can affect blood vessels, nerves or several ligaments even when the PCL is the known tear.
Arrange an orthopedic review when repeated giving-way, persistent swelling, locking or loss of motion continues despite the initial plan. These features may indicate instability or another injured structure that changes management.
