How vaginismus works
Vaginismus involves involuntary tightening of the muscles around the vaginal opening when penetration is expected or attempted. It can make sex, tampon use, or an examination painful or difficult. The response is real and does not mean you are choosing to resist. Pain during sex can have other causes too, so an assessment helps identify infection, skin conditions, hormonal changes, pelvic pain, or another explanation.
Building progress safely
Treatment is usually paced around your comfort. Pelvic floor physiotherapy can teach relaxation, breathing, awareness, and coordinated muscle release. Dilator therapy uses graduated devices only when you feel ready, with lubrication and no force; a clinician can show you how to begin. Counselling or sex therapy may help with fear, past pain, relationship communication, or body confidence. Stop if pain escalates and tell your care team what happened.
When to seek care
Arrange an assessment if penetration is painful, impossible, or becoming more difficult, especially with bleeding, unusual discharge, sores, pelvic pain, or urinary symptoms. Seek prompt care for sudden severe pelvic pain, fever, fainting, or heavy bleeding. You do not need to force an examination; ask about a trauma-informed approach and agree on each step beforehand.
Questions for your doctor
Ask what may be contributing to the muscle response, whether another condition needs checking, which therapy fits your goals, how progress will be measured, and what to do if an exercise causes pain.

Questions people ask
Can vaginismus be treated?
Many people improve with a combination of pelvic muscle treatment, gradual exercises, and support for pain or emotional concerns.
Should I push through pain during dilator therapy?
No. Pain is a signal to pause, reduce the step, and review technique with your clinician.
