The short answer

It often follows a force that pushes the knee inward or a twisting incident. Inner-knee pain and tenderness are common; swelling, stiffness, and a sense that the knee may give way can occur when the injury is more substantial.

What can be injured

The medial collateral ligament connects the thigh bone to the shin bone along the knee's inner side. A mild sprain stretches some fibres, while a more significant injury disrupts more of the ligament and can make side-to-side stability less reliable.

An MCL can be injured alone, but a forceful twist or collision may also affect cartilage or other knee ligaments. Examination focuses on the pain location, range of motion, swelling, and stability. Imaging may be considered when the examination suggests another injury or a fracture.

Protecting the knee and rebuilding function

Stop the activity that caused pain and avoid cutting, pivoting, or contact movements until the knee has been assessed. Relative rest, a wrapped cold pack, comfortable elevation, and protection from further knocks may help early symptoms. A clinician can advise whether a brace or walking aid is appropriate.

Rehabilitation usually progresses from restoring comfortable movement to strengthening the thigh and hip, then balance, running, direction changes, and sport-specific tasks. Return should be based on control, stability, and task tolerance rather than the calendar alone.

When to arrange prompt help

Seek urgent assessment after a major injury if the knee looks deformed, you cannot bear weight, the foot becomes cold or numb, or pain and swelling are severe. A locked knee that cannot fully bend or straighten also needs prompt attention.

Book a clinical review if the knee repeatedly gives way, swelling persists, inner-knee pain is not improving, or ordinary walking remains difficult. Continuing sport on an unstable knee may expose it to another injury.

What to clarify at your assessment

Describe the direction of the force, any pop, how quickly swelling appeared, and whether the knee buckles or locks. Ask which structures appear involved, whether you need a brace, what movements are currently safe, and what functional checks should guide work or sport clearance.
