What is happening in the bowel

Ulcerative colitis affects the inner lining of the colon and rectum. Inflamed lining can produce frequent loose stools, urgency, mucus or blood, while cramps may occur before a bowel movement. Symptoms can be active for a time and then become quieter, although the pattern varies between people.

The condition is assessed from your symptoms, examination and appropriate tests. A diagnosis is not made from one symptom alone. Other causes of diarrhoea, bleeding or pain can look similar, which is why a clinician may recommend stool tests, blood tests or a camera examination of the bowel.

Managing the next step

Write down bowel changes, bleeding, pain, fever, appetite and weight change. Take your prescribed medicines consistently and contact your care team if they are not controlling symptoms or cause troublesome effects. During a flare, fluids and an agreed eating plan may help, but avoid making major dietary restrictions without advice.

Arrange a GP Consultation for persistent, new or changing symptoms. Your clinician can decide whether specialist review or colorectal testing is needed. Colorectal Cancer Screening may be offered as planned preventive care, while active symptoms may require a different diagnostic assessment.

Use urgent services for severe abdominal pain, a markedly swollen abdomen, repeated vomiting, heavy bleeding, faintness or inability to keep fluids down. Early review can distinguish a flare from infection or another problem.
