Colles fracture reduction: what patients can expect

A Colles fracture is a distal radius fracture in which the broken end near the wrist has shifted. During closed reduction, a clinician moves the fragments into a more suitable position without making a surgical incision. Pain control or anaesthesia is provided according to the setting and patient. Imaging is commonly used to inspect the position, and the wrist is then supported. If acceptable alignment cannot be obtained or maintained, fracture fixation surgery may be considered.

Keep the splint or cast dry and follow the instructions on elevation and finger movement. Remove rings early if the treating team advises, because hand swelling can make them constrictive. Do not insert objects inside the cast or alter it yourself. Attend scheduled imaging and fracture-clinic visits even when pain is improving, since bone position and healing still need review. Avoid lifting, gripping forcefully or leaning through the injured hand until cleared. Rehabilitation can later restore wrist motion, forearm rotation and strength.

Return urgently if the fingers become cold, blue or very pale, or if new numbness, worsening tingling, marked weakness or inability to move them develops. Severe or rapidly increasing pain and swelling, especially when elevation does not help, may mean the support is too tight or pressure is building. Seek advice for a cracked, loose, wet or foul-smelling cast, and for fever, spreading redness or wound discharge after a procedure. A new fall onto the wrist also warrants reassessment.

Find out whether the post-reduction position is acceptable, how to protect the arm during sleep and washing, and which finger or shoulder movements are encouraged. Ask when the next alignment check is planned, what might lead to another reduction or surgery, and when hand therapy should start. Clarify how to use prescribed pain medicine and whom to contact if the cast becomes uncomfortable.
