Cold urticaria causes: what can trigger it?

The immune system releases inflammatory chemicals in the skin after cooling. This can produce raised, itchy welts within minutes, especially where cold touched the body. Warming the skin may make the reaction more noticeable as circulation returns.

The trigger may be a cold drink touching the lips, swimming in cool water, holding a chilled item, or being outside in cold weather. Wind can increase cooling even when the air temperature does not feel extreme. Symptoms can include itchy skin, a red or blotchy rash, and swelling of the hands, lips or other exposed areas.

Some cases begin without an obvious family history or separate illness. In other cases, cold urticaria occurs alongside an infection, an immune-related condition or another medical problem. A clinician may review your history and examine the reaction to decide whether further evaluation is appropriate. The condition is different from ordinary dry skin caused by cold weather.

Keep the skin warm and avoid sudden, extensive cold exposure until you have discussed the pattern with a healthcare professional. Do not test your reaction by jumping into cold water: a large-area exposure can cause a stronger response than a small patch of cold skin.

Record what caused the episode, how quickly it started, which body areas changed, and how long it lasted. A clinician may recommend an oral antihistamine treatment to reduce itching and welts. Do not start oral corticosteroid treatment without medical advice; it is not suitable for every episode.

Seek emergency help immediately for trouble breathing, wheezing, faintness, confusion, a weak pulse, or swelling of the tongue or throat after cold exposure. These symptoms can signal a severe whole-body reaction, particularly after swimming or another large-area exposure.

Arrange medical assessment for repeated episodes, swelling that affects eating or speaking, or symptoms that occur without a clear cold trigger. Bring a list of medicines and a description or photograph of the rash if it has already faded.

Ask which exposures you should avoid, whether your medicines could affect the reaction, and what to do if swelling spreads beyond the original cold-exposed area. You can also ask whether you need an emergency action plan for activities such as swimming or travel to a colder climate.
