Cold limbs with fever: what to know

When a fever is developing, the body may reduce blood flow to the skin and cause shivering, so hands and feet feel cold even while the core temperature is raised. Viral illnesses such as the common cold, COVID-19, bronchiolitis, croup, or chickenpox can cause fever with other symptoms. Acute bronchitis may add a cough. A skin infection such as cellulitis usually causes a local area of warmth, redness, swelling, and tenderness rather than uniformly cold limbs. Travel or animal exposure can be relevant to illnesses such as brucellosis.

Check the person's overall condition, not just the temperature. Drinking, alertness, breathing, skin colour, and urine output help show whether illness is becoming more serious.

Let the person rest, offer frequent fluids if they are awake and able to swallow, and use light clothing and bedding. During chills, a light blanket may help comfort, but avoid overheating. Follow the label or a clinician's instructions for any fever medicine, and never give a child a medicine that is unsuitable for their age. Do not rub cold hands or feet vigorously or use direct high heat.

If the fever persists, returns, or is accompanied by cough, rash, vomiting, pain, or reduced drinking, contact a clinician. Keep track of when the fever began, associated symptoms, medicines taken, and any recent travel or animal contact.

Seek emergency help for difficulty breathing, blue or grey lips or skin, new confusion, fainting, a seizure, severe drowsiness, or a rapidly worsening condition. Get urgent advice if limbs remain cold and pale or mottled after chills settle, or if the person cannot keep fluids down. Babies, older adults, pregnant people, and anyone with significant long-term illness may need earlier assessment.

Ask what may be causing the fever, whether the limb temperature and colour are concerning, which fluids or fever medicines are appropriate, and what change should prompt immediate reassessment.
