Cold, fever and rash: what to know

Respiratory viruses may cause fever and cold symptoms, and some also cause a rash. Chickenpox often brings an itchy blistering rash in different stages. Cellulitis tends to cause a local area of redness, warmth, swelling, and tenderness rather than a widespread cold-like rash. COVID-19, common cold viruses, bronchiolitis, croup, and acute bronchitis can have overlapping symptoms. A rash after a new medicine may be a reaction. Note whether spots fade when pressed, blister, peel, hurt, itch, or involve the mouth or eyes.

Take a clear photo in good light and record when the fever and rash began. Encourage fluids and rest, avoid scratching, and keep the skin clean. Do not share towels, cups, or clothing if an infectious rash is possible, and ask a clinician about staying home from school or work. Avoid starting antibiotics or using a steroid cream without advice, because treatment depends on the cause and some rashes need examination.

Get emergency help for difficulty breathing, swelling of the lips or face, collapse, confusion, severe headache with a stiff neck, or a purple or bruise-like rash that does not fade with pressure. Seek same-day medical advice for a rapidly spreading, painful, blistering, or peeling rash, eye or mouth involvement, dehydration, or fever in a very young infant. Prompt review is also sensible when the person is pregnant or immunocompromised.

Ask whether the rash looks viral, bacterial, allergic, or medicine-related; whether testing or treatment is needed; and how long to avoid close contact. Share recent medicines, travel, sick contacts, vaccination history, insect bites, and photos of how the rash changed.
