Understanding the pattern

When the body is adjusting its temperature set point, blood vessels in the skin can narrow and cause cool hands or feet. Shivering may follow, even when the measured temperature is high. Common infections can produce this pattern, including a common cold, COVID-19, acute bronchitis, or croup. Bronchiolitis can be particularly concerning in babies because breathing effort may increase. Cellulitis may cause a local hot, red, tender area rather than generally cold feet. The diagnosis depends on the whole symptom pattern, not the foot temperature alone.

Comfort and monitoring

Give small, regular drinks and allow quiet rest. Use breathable clothing, a light cover, and ordinary socks if the feet are uncomfortable; avoid ice baths and very hot water. Measure temperature consistently and write down medicines, times, fluid intake, urine, breathing effort, and new symptoms. Use fever medicine only as directed for the person's age and health circumstances. Check children regularly, including during sleep if they seem significantly unwell.

When to seek care

Call emergency services for severe breathing difficulty, blue or grey colouring, a seizure, collapse, confusion, or inability to wake the person. Seek urgent assessment for cold clammy skin, a rapidly spreading rash, severe pain, repeated vomiting, signs of dehydration, or a child who is not drinking or urinating normally. Medical advice is also appropriate when fever persists, returns after improvement, or affects someone medically vulnerable.
