Traumatic coccydynia begins after a direct impact, while postpartum coccydynia appears after childbirth-related strain. Repetitive-pressure pain develops with prolonged sitting or activities that repeatedly load the coccyx. In some people, the coccyx moves more or less than expected during sitting and rising; clinicians may call this dynamic or mobility-related pain.

Persistent symptoms may be described as chronic, whereas a recent episode is acute. Idiopathic coccydynia means that no definite cause is found after assessment; it does not mean the pain is imaginary. These labels overlap and describe a clinical pattern rather than separate diseases with a single test for each.

How the pattern changes next steps

Record when the pain began, any fall or childbirth connection, the seats and movements that trigger it, and whether there is a lump or skin change. Reduce direct pressure with a coccyx cut-out cushion, avoid remaining in one position, and keep comfortable activity going rather than repeatedly testing the sore point.

When pain persists, an assessment can separate coccyx tenderness from hip, lower-spine, pelvic-floor, or skin problems. Physiotherapy is more useful when it is matched to the findings, such as restricted movement, poor load distribution, or muscle guarding. A procedure should target a reasonably identified pain source rather than a label alone.

Warning signs that need separate evaluation

Fever, warmth, redness, drainage, an enlarging swelling, or progressive pain at rest may point away from uncomplicated coccydynia and needs prompt care. Seek urgent help after substantial trauma if mobility is impaired. New leg weakness, altered feeling around the groin, or loss of bladder or bowel control requires emergency assessment regardless of the suspected type.

Ask which examination finding links your symptoms to the coccyx, whether the proposed label changes treatment, and whether another source of pain has been considered.
