Cluster headache in both eyes: what to know

A cluster attack causes severe pain around or behind one eye and may bring tearing, redness, a blocked or runny nostril, eyelid swelling, or restlessness on that side. The pain often starts suddenly and comes in repeated attacks during an active period. Symptoms involving both eyes at once are less typical. They can reflect another headache pattern, an eye problem, or a condition affecting the nerves, so the pattern matters more than the label alone. Note which eye is affected, whether vision changes, and whether redness or swelling continues between attacks.

Arrange a prompt clinical review, especially if this is a first severe attack or both eyes are involved. Record the start time, duration, side, eye changes, nasal symptoms, triggers, and medicines taken. Rest in a quiet, dim setting while waiting, and avoid driving if vision is blurred or pain is distracting. A clinician may distinguish cluster headache from other causes and discuss fast-acting treatment or prevention when appropriate.

Seek emergency help for sudden severe pain with loss of vision, a new unequal pupil, weakness, confusion, fainting, trouble speaking, fever with a stiff neck, or a painful red eye with vomiting.
