Cluster headache causes and origins

Etiology means the origin or cause of a condition. Cluster headache appears to involve the hypothalamus, a brain region that helps regulate sleep and daily rhythms, alongside activation of the trigeminal pain system and nearby automatic nerve pathways. This helps explain severe pain around one eye, tearing, nasal blockage or facial sweating on the same side.

Attacks often arrive in patterns, with active periods separated by quieter intervals. Alcohol can trigger an attack during an active period for some people, while disrupted sleep or a changing routine may also matter. A trigger is not the same as the underlying cause, and many people have no clear trigger.

Write down when attacks begin, how long they last, which side hurts, associated eye or nose symptoms, sleep timing, possible triggers and all medicines. A GP can review the pattern, examine you and decide whether specialist assessment or imaging is appropriate, especially if the pattern is new or unusual. Avoid deliberately testing suspected triggers. Bring a list of medical conditions and medicines, including non-prescription products.

Seek prompt care for a first severe attack, a major change in an established pattern, headache after injury, or pain with fever, stiff neck, confusion, weakness, fainting or persistent vision change. Emergency assessment is needed for sudden explosive headache or new neurological symptoms. A familiar cluster pattern still deserves a treatment plan because repeated attacks can be disabling.
