The short answer

A fine crackle, distinct click and coarse grind are different descriptions, but sound quality cannot confirm the cause. The practical distinction is whether the shoulder remains comfortable, strong and mobile or has stiffness, catching, weakness or a sense of instability.

Possible sources of rough movement

Noise may come from tendons changing direction, the shoulder blade gliding over the chest wall, or surfaces within a joint moving against each other. Prior injury, prolonged stiffness and altered shoulder-blade mechanics can change how these tissues travel during reaching.

Shoulder osteoarthritis commonly presents with painful stiffness and can include grinding as joint surfaces become less smooth. Shoulder instability can present with clicks, clunks and slipping sensations, particularly after dislocation. Neither condition should be inferred from noise in isolation.

Managing a noisy, irritated shoulder

Temporarily modify motions that combine grinding with pain. Keep the shoulder moving within an easy range rather than holding it completely still, and divide demanding overhead work into manageable bouts. When lifting, choose a load that allows steady control without shrugging or forcing through a catch.

An assessment can compare motion on both sides, locate tenderness, measure strength and check whether the joint shifts excessively. Rehabilitation may focus on mobility where movement is restricted and control where the shoulder blade or upper arm moves inefficiently. Treatment should address the functional finding, not the noise alone.

When grinding warrants a check

Seek a routine evaluation if grinding is progressively painful, movement is shrinking, sleep is disturbed, or everyday tasks become difficult. Recurrent catching, a slipping joint or continuing symptoms after an injury are also concrete reasons for an appointment.

Urgent assessment is appropriate for visible deformity, inability to move the arm after trauma, new marked weakness or lasting numbness. Go promptly for fever with a hot swollen shoulder, or if the hand becomes pale or cold, as these signs require more than activity modification.

Should I rest a grinding shoulder completely?
