The short answer
Clicks can occur as the arm rises, rotates or returns to the side. Their timing, exact location and association with pain, catching, weakness or slipping give a clinician more useful information than whether the noise sounds loud or sharp.
Reading the movement pattern
A click near the top of the shoulder may arise around the collarbone joint, while a deeper sensation can come from structures around the ball-and-socket joint. Tendons can move over nearby contours, and the shoulder blade can create sound as it glides along the rib cage. Location by itself is not a diagnosis.
Shoulder instability often presents with more than noise, such as repeated slipping, apprehension in certain positions or a history of dislocation. Shoulder osteoarthritis can present with deeper clicking or grinding accompanied by painful stiffness. Examination separates these patterns from comfortable mechanical sounds.
How to respond to the sound
First identify the triggering movement without repeatedly testing it. For a painful click during lifting, use a lighter resistance, shorten the range and keep the elbow in a comfortable path. Allow irritated tissues to settle before rebuilding load in small increments.
If the sound is painless and function is unchanged, ordinary use can continue. When symptoms persist, a physiotherapist can assess shoulder-blade control, rotation, strength and joint stability. A doctor may consider imaging when trauma, examination findings or sustained loss of function suggests it would clarify treatment.
When a click is not the only issue
Make an appointment when the sound repeatedly coincides with pain, locking, night symptoms, weakness or reduced reach. Review is also appropriate if the shoulder feels as though it may come out, especially after a previous dislocation.
Following injury, seek prompt assessment if you cannot move the arm normally, the shoulder contour changes, or numbness persists. Fever with a red, hot joint, a cold hand, or rapidly increasing pain and swelling calls for urgent medical care.

Tissues and the shoulder blade change position as the arm descends. The phase of movement is a useful examination clue, but it does not identify one structure by itself.
