Claudin-4 immunohistochemistry: understanding your pathology result

A positive or negative result is a clue, not a stand-alone diagnosis. The pathologist interprets where the stain appears, how strong and widespread it is, the cells' microscopic features, and the results of other markers.

Claudin-4 is a protein associated with tight junctions between many epithelial cells. A report describing membranous positivity means the stain outlines cell borders. That pattern can support carcinoma in the right setting and may help separate it from mesothelial tumors or some other tumor categories.

Expression varies across tumors, and a sample can contain too few abnormal cells or areas with different staining. A negative stain therefore does not automatically exclude carcinoma, while a positive stain does not reveal the tumor's organ of origin by itself. Broader marker panels and the tissue appearance narrow the possibilities.

Read the claudin-4 line alongside the final diagnosis and comment section rather than interpreting the word positive in isolation. Find out whether the stain was performed on tumor tissue, whether internal control cells stained as expected, and which other markers support or challenge the pathologist's interpretation.

If the report remains indeterminate, the clinical team may compare prior samples, obtain additional stains, request specialist review, or consider another biopsy. The appropriate choice depends on sample quality, the suspected tumor type, imaging findings, and whether a more specific diagnosis would change management.

The stain result itself is not an emergency. Follow the care instructions for the condition being investigated and for the biopsy site. Contact the treating team promptly for increasing redness, warmth, swelling, drainage, fever, worsening pain, or bleeding that does not settle after the procedure. Seek urgent care for severe shortness of breath, fainting, confusion, or heavy bleeding.

You can ask what staining pattern was seen, which diagnosis the result supports, what alternative diagnoses remain, and whether the sample was adequate. Clarify whether more stains or tissue are needed, when the integrated pathology report will be available, and how that conclusion affects the next clinical decision.
