What chronic SCFE means
SCFE stands for slipped capital femoral epiphysis. It affects a growing hip: the ball at the top of the thighbone shifts relative to the neck of the bone through the growth plate. “Chronic” describes symptoms that have developed gradually rather than after one sudden event.
A child may develop a limp, turn the foot outward, or report pain in the hip, groin, thigh or knee. The hip can become stiff, especially when turning the leg inward. Knee pain can distract attention from the hip, so examination usually includes both areas. X-rays help confirm the slip and guide treatment planning.
What to do next
Arrange urgent medical assessment if SCFE is suspected. Stop running, sport and walking on the painful leg; do not try to stretch, manipulate or “put the hip back.” The usual aim of surgery is to secure the femoral head and prevent further slipping. The exact operation depends on stability and the shape of the hip.
After treatment, follow the surgeon’s instructions on wound care, weight bearing, school and activity. Rehabilitation is introduced when the hip is stable enough. Follow-up checks movement, healing and the other hip, because management needs to be tailored to the child’s growth and individual findings.
When to seek urgent care
Seek urgent care now if a child with hip, thigh or knee pain cannot stand or bear weight, develops a suddenly worse limp, or has a sharp increase in pain. These changes can indicate an unstable or worsening slip and should not wait for a routine appointment.
After surgery, contact the treating team promptly for fever, increasing wound redness, drainage, uncontrolled pain, new numbness, a cold or pale foot, or a sudden loss of movement.

Will physiotherapy alone correct chronic SCFE?
