The short answer

It can affect one bone or several areas and may alternate between active episodes and quieter periods. Although its name resembles infectious osteomyelitis, antibiotics do not treat the underlying sterile inflammation.

What the diagnosis means

Pain, tenderness and swelling can occur over an affected bone. Some people also develop reduced movement near a nearby joint. The pattern varies, so diagnosis may involve an examination, blood tests and imaging, while checking for infection, injury or another bone disorder.

Doctors may use the term chronic recurrent multifocal osteomyelitis when inflammation returns and involves multiple sites. The condition is considered autoinflammatory: immune activity causes inflammation even though bacteria are not driving it.

How to manage it

Keep a record of painful areas, swelling, night discomfort, activity limits and any skin or bowel symptoms. This helps the treating team see whether inflammation is moving, recurring or responding to care.

Management may include anti-inflammatory medicine, other medicines that modify inflammation, and physiotherapy to preserve comfortable movement and strength. Treatment is tailored to the bones involved and the impact on daily life. Do not start leftover antibiotics; infection must be assessed separately.

When to seek medical care

Arrange a clinical review for persistent focal bone pain, repeated swelling, a new limp or symptoms that interrupt sleep. Seek urgent assessment for fever with a hot, rapidly worsening swollen area, severe pain after an injury, inability to bear weight, or feeling acutely unwell, because infection or fracture needs prompt exclusion.
