The short answer
The painful spot may not be the structure causing the problem: the hip joint, adductor tendons, abdominal wall, lower back, nerves, urinary tract, and reproductive organs can all refer pain toward the groin.
What the pain pattern can reveal
Hip impingement and a hip labral tear can present with groin pain provoked by deep bending, pivoting, or prolonged sitting; clicking or catching may occur. Hip osteoarthritis can present with groin pain accompanied by stiffness and reduced movement. These patterns help focus an assessment but do not confirm a diagnosis.
Pain during sprinting, kicking, squeezing the knees, or changing direction can point toward muscles or tendons around the inner thigh. A bulge that changes with coughing or straining raises concern for a hernia. Burning, tingling, urinary symptoms, menstrual or pelvic symptoms, and testicular or scrotal changes direct attention to other systems. More than one source can contribute at the same time.
What to do next
Book a clinical assessment if pain keeps returning, limits walking or sport, disturbs sleep, or has not settled with sensible activity changes. Note where the pain begins, which movements reproduce it, whether it travels, and any clicking, bulge, back pain, urinary change, or genital symptom.
Until assessed, reduce movements that clearly flare the pain while keeping comfortable general movement. Avoid repeatedly forcing a deep stretch or testing a painful manoeuvre. A clinician may examine the hip, abdomen, back, and nearby muscles and may recommend imaging or other tests based on the findings. Physiotherapy can use graded loading, strength, movement control, and a planned return to activity when a musculoskeletal source is identified.
When groin pain needs urgent care
Seek emergency assessment for sudden severe testicular or scrotal pain, especially with swelling, nausea, or vomiting. A painful groin bulge that cannot be gently reduced, particularly with vomiting or abdominal swelling, also needs urgent assessment.
Get prompt care for fever with redness or warmth, rapidly increasing swelling, inability to bear weight after an injury, blood in the urine, or feeling acutely unwell. New numbness around the groin with leg weakness or loss of bladder or bowel control requires emergency evaluation. Urgency is based on these warning signs, not on how long mild symptoms have been present.
