What should you know about cholestasis and itching?

Cholestasis means bile flow is reduced or blocked. Substances normally handled through bile can build up and contribute to intense itching, often on the hands, feet, arms, or across the body. Scratching may leave marks, but the original skin can look fairly normal. Yellowing of the skin or eyes, dark urine, pale stools, abdominal discomfort, nausea, or unusual tiredness can provide important clues. Dry skin, eczema, contact dermatitis, psoriasis, scabies, hives, and other conditions can also itch, so appearance alone cannot confirm cholestasis.

Arrange medical assessment rather than treating persistent generalized itching only with moisturiser. A clinician may review medicines, pregnancy status, alcohol and health history, examine the skin, and order blood tests or imaging when needed. Use fragrance-free moisturiser, lukewarm water, loose clothing, and short nails to reduce skin injury while waiting. Do not start supplements, bile remedies, or sedating medicines without checking first, because they may interact with treatment or obscure important symptoms.

Seek prompt medical care for new yellow skin or eyes, severe or worsening abdominal pain, fever, confusion, repeated vomiting, bleeding, or very dark urine with pale stools. During pregnancy, new intense itching—especially on the palms or soles—needs timely contact with the maternity team. Emergency help is needed for faintness, breathing difficulty, or sudden facial or throat swelling.
