What should you know about a first cholera outbreak?

Short answer
During a first cholera outbreak, sudden watery diarrhoea can cause rapid fluid loss, so start rehydration and seek medical assessment promptly.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Main risk
- Rapid loss of water and salts through diarrhoea and vomiting
- First response
- Start oral rehydration and arrange prompt medical assessment
- Spread prevention
- Safe water, careful food handling, and thorough handwashing
What a first cholera outbreak means
Cholera is an intestinal infection caused by bacteria that can enter the body through contaminated water or food. In an outbreak, several people in the same area may become ill after exposure to the same unsafe source. A cholera infection commonly presents with sudden, profuse watery diarrhoea; vomiting may add to the fluid loss. Some infected people have mild illness or no symptoms, but they can still contaminate hands, food, or water if hygiene is poor.
The main danger is dehydration rather than the diarrhoea alone. Warning features include intense thirst, dry mouth, weakness, dizziness, reduced urination, sunken eyes, confusion, or unusual sleepiness. Young children, older adults, pregnant people, and anyone already unwell can deteriorate quickly. Cholera cannot be confirmed from symptoms alone, so a clinician or public-health team may arrange testing and advise on local precautions.
What to do now
Begin Oral Rehydration Therapy as soon as possible, following the product instructions and using safe water. Take frequent small sips; if vomiting occurs, pause briefly and try again more slowly. Keep drinking while arranging medical advice. Severe dehydration may require IV Fluid Therapy in a healthcare setting, and a clinician will decide whether antibiotics or another treatment is appropriate.
Use soap and water to wash hands after using the toilet and before preparing food. Use treated or bottled water for drinking, brushing teeth, and making rehydration solution. Cook food thoroughly, avoid raw foods that may have been washed with unsafe water, and clean toilets and frequently touched surfaces. Do not prepare food for other people while unwell.
When to seek urgent care
Seek urgent medical care for repeated watery stools, inability to drink, ongoing vomiting, faintness, confusion, marked weakness, very little urine, or signs of dehydration. Get help immediately if a child becomes unusually sleepy, cannot drink, or has sunken eyes. Tell the service that cholera may be possible and describe recent travel, shared meals, unsafe water exposure, and anyone else who is ill. Keep sipping rehydration fluid while help is being arranged.
Questions for your doctor
Ask whether testing is needed, how much rehydration fluid to take, which water sources are safe locally, and whether household members need advice or monitoring. Ask how to recognise worsening dehydration and when it is safe to return to work, school, or food preparation.

Questions people ask
Can cholera be treated?
Yes. Replacing lost fluids and salts is the central treatment. Some people need IV fluids, and a clinician may prescribe additional treatment based on the illness and local guidance.
Can I prevent cholera spreading at home?
Reduce spread by washing hands with soap and water, using safe water, cooking food thoroughly, cleaning toilets and surfaces, and avoiding food preparation for others while you are unwell.
Is every episode of watery diarrhoea cholera?
No. Many infections and other conditions can cause watery diarrhoea. Testing and clinical assessment help identify the cause, while rehydration should begin promptly when fluid loss is suspected.
