Cheilitis and swollen lips: what to know

Swollen lips can occur with cheilitis, but sudden or rapidly worsening swelling may signal angioedema and needs urgent attention. Cheilitis more often includes surface changes such as dryness, scaling, burning, or cracks. Swelling can also follow food, medicine, an insect sting, injury, infection, or contact with a product, so timing and accompanying symptoms are useful clues.

Angioedema is deeper swelling that may affect a lip, eyelid, tongue, or throat and can occur with or without hives. Surface redness and scaling suggest inflammation at the skin barrier, while warmth, worsening pain, pus, or fever can suggest infection. Take a photograph and note new foods, medicines, cosmetics, and dental products.

Stop any newly introduced lip or facial product and avoid rubbing the area. If swelling is mild and stable, use a cool compress through a clean cloth and arrange clinical advice, especially if it is a first episode. Do not apply medicated creams or take someone else’s medicine. Keep a record of the onset and any exposures.

Call emergency services immediately for tongue or throat swelling, breathing difficulty, wheezing, faintness, trouble swallowing, or rapidly progressing swelling. Urgent assessment is also needed when swelling comes with widespread hives, severe pain, fever, or spreading redness. Angioedema can progress even when the skin surface looks relatively normal.
