Recognising a night-time cramp

A sleep-related cramp commonly feels like an abrupt tightening in the calf or foot, sometimes with a firm knot under the skin. It differs from a muscle contusion, which follows a direct impact and is more likely to leave focal tenderness, swelling, or discolouration.

Night cramps can occur without a clear cause. Muscle fatigue, prolonged positioning, fluid loss, pregnancy, some health conditions, and certain medicines can contribute. The timing alone cannot identify the cause.

Respond during the cramp and prepare for the next night

For a calf spasm, straighten the knee within comfort and pull the toes toward the shin. Hold a bedside support before standing, then walk gently after the muscle loosens. Do not bounce into the stretch or push through sharp pain.

During the day, maintain comfortable activity, warm up before demanding exercise, and drink according to thirst and conditions. Review whether episodes followed unusually hard activity or a medicine change. Keep the route from bed clear to reduce the chance of a fall during a sudden awakening.

When night cramps need medical attention

Book an assessment if cramps repeatedly disturb sleep, are becoming more frequent, or occur with lasting weakness, numbness, or swelling. Seek urgent care for one-sided calf swelling with redness or warmth, severe pain after injury, or a cold or pale foot. Sudden shortness of breath or chest pain requires emergency help.
