What to Know About a Charley Horse in the Inner Thigh

How the structures involved in Muscle Contusion differ from normal

Short answer

An inner-thigh charley horse is often a sudden adductor muscle cramp, but the term can also describe a muscle bruise after impact.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Muscles involved
The adductor group along the inner thigh
Cramp clue
A sudden involuntary knot that later releases
Bruise clue
Pain after direct impact with tenderness, swelling, or discolouration

Direct answer

A cramp makes the muscle tighten involuntarily and may create a hard, painful knot. A bruise is associated with a blow or collision and can produce longer-lasting tenderness, swelling, or skin discoloration. A strain is another possibility if pain began during a forceful push, sprint, or change of direction.

How to interpret inner-thigh pain

The adductors run along the inner thigh and pull the leg toward the body's midline. Fatigue, unfamiliar exercise, prolonged muscle shortening, heat, or fluid loss may set up a cramp. A contusion results from compressed muscle tissue after a direct hit, while a strain involves overstretched or torn fibres.

Notice whether the muscle visibly bunches, whether bruising appears, and whether walking or squeezing the knees together provokes pain. Groin, hip, nerve, or circulation problems can produce symptoms in a similar area, so location alone cannot establish the diagnosis.

Relief and recovery

During a cramp, stop, support yourself, and slowly move the leg outward until the inner thigh lengthens gently. Ease off if stretching sharpens the pain. Light walking may help after the contraction releases, and replacing fluids can be reasonable after sweating.

If a collision caused the pain, protect the thigh and apply wrapped cold rather than pressing hard into it. Avoid aggressive stretching and sports massage while swelling or bruising is fresh. Build back through comfortable walking, easy range-of-motion work, and then sport-specific movement.

Signs that need assessment

Get urgent medical help if one leg becomes notably swollen, warm, or discoloured without a clear minor injury, especially with chest pain, breathlessness, coughing blood, or faintness. Seek prompt care after major trauma, or if you cannot bear weight, the thigh looks deformed, or pain is escalating.

Book a clinical review when cramps recur, weakness persists, bruising expands, numbness develops, or everyday walking remains limited. An examination can distinguish cramp, contusion, strain, and pain referred from the hip or groin.

Points to discuss at an appointment

Describe the movement or impact that started the pain, any visible muscle tightening, and what now limits you. Ask whether rehabilitation should focus on adductor flexibility, strength, hip control, or a staged return to your activity, and whether your medicines or health history could contribute to repeated cramps.

Questions people ask

Is an inner-thigh cramp the same as a groin strain?

No. A cramp is an involuntary contraction that releases, whereas a strain is an injury to muscle fibres and commonly remains painful with movement afterward.

Can I massage the tight area?

Gentle rubbing may feel comfortable after a simple cramp releases. Avoid forceful massage if there was an impact, significant bruising, swelling, or an undiagnosed painful lump.

Why does it return during sport?

Muscle fatigue, rapid direction changes, an abrupt rise in training load, or incomplete recovery may contribute. Repeated episodes deserve a movement and health assessment.
