Cervical ectropion during pregnancy

It occurs when delicate glandular cells from inside the cervical canal are visible on the outer surface of the cervix. Pregnancy-related hormonal changes can make this area more prominent and easier to bleed when touched. It is not the same as cervical cancer.

Many people notice no symptoms. Cervical ectropion can present with increased clear or mucus-like discharge and light spotting after sex, a cervical swab or an examination. These features can have other causes, so ectropion should not be diagnosed from symptoms alone.

A clinician may see the characteristic red, delicate area during a speculum examination. Depending on the situation, assessment can also include swabs for infection and checking whether cervical screening is due. Screening is not a test for the cause of every episode of pregnancy bleeding.

Report any new bleeding to your maternity team so they can decide what assessment is needed. Note the colour and amount, whether it followed sex or an examination, and whether pain, tightening, fluid leakage, itching, odour or urinary symptoms occurred with it.

When ectropion is confirmed and symptoms are mild, monitoring is often appropriate. Use a pad rather than a tampon to observe bleeding, avoid vaginal douching and follow individual advice about sex if contact repeatedly triggers spotting. Treatment of the cervical surface is generally considered only after clinicians exclude other causes and decide it is appropriate during pregnancy.

Seek urgent maternity assessment for heavy or increasing bleeding, severe or one-sided abdominal pain, dizziness or fainting, regular contractions, fever, a gush or continuing trickle of fluid, or reduced fetal movement once a usual pattern is established. Do not assume bleeding is from ectropion simply because the condition was seen before.

Ask what else could explain your symptoms, whether you need an examination or infection testing, and whether your cervical screening schedule is up to date. Clarify how to respond if spotting returns, whether any activity changes are advised and which maternity service to contact after hours.
