Cervical and lumbar radiculopathy explained

Cervical radiculopathy begins at a nerve root in the neck and can produce symptoms across the shoulder, arm or hand. Lumbar radiculopathy begins in the lower back and may travel through the buttock into the thigh, calf or foot; irritation involving the sciatic pathway is often called sciatica. A herniated disc, age-related narrowing around a nerve, spinal stenosis or vertebral slippage can contribute.

The symptom pattern helps identify the involved nerve, but an examination is needed because joint, muscle and peripheral nerve problems can overlap. A clinician may compare strength, sensation and reflexes on both sides and decide whether imaging or nerve tests would change care.

Arrange an assessment if radiating pain, altered sensation or weakness persists, interferes with sleep or limits normal activity. Until then, use comfortable positions, change posture regularly and keep gently active within tolerable limits. Avoid movements that clearly intensify spreading pain or weakness. Treatment may include guided exercise, activity modification and medicines selected for your health history; procedures or surgery are considered when the cause, severity and response to simpler care support them.

Seek urgent medical care for new loss of bladder or bowel control, numbness around the groin or saddle area, rapidly worsening limb weakness, major difficulty walking, or symptoms after significant trauma. Fever with severe spinal pain, or pain accompanied by feeling systemically unwell, also warrants prompt assessment. New hand clumsiness, balance trouble or symptoms affecting several limbs may indicate spinal cord involvement rather than a single nerve root.

Ask which nerve root seems involved, what findings support that view, whether weakness is present, and what activities are suitable now. You can also ask what change would justify imaging, how progress will be monitored and which warning signs should alter the plan.
