What should you know about certified hand therapist certification?

Certification requirements differ between credentialing bodies and countries. A therapist's base profession may be occupational therapy or physiotherapy, and the permitted scope of practice follows local regulation.

Hand therapists assess movement, swelling, sensation, strength, pain, scars, and the ability to perform daily tasks involving the hand, wrist, forearm, or elbow. Care may include tailored exercises, education, activity modification, scar management, and fabrication or adjustment of a splint.

They often support recovery after a fracture or operation and may treat tendon, nerve, joint, or soft-tissue problems. They do not replace the surgeon or physician when a new diagnosis, medication decision, imaging, or urgent medical treatment is required.

Check the therapist's current professional licence and the organization that issued the specialist credential. Ask whether they regularly manage your diagnosis, operation, work demands, sport, or splinting needs, and whether they coordinate with your referring clinician.

Bring referral notes, imaging reports if available, your medication list, and any splint already in use. Explain which tasks are difficult and what improvement matters most to you. Follow wound, movement, and loading restrictions from the surgical team until those instructions are formally changed.

Seek urgent assessment after an injury if a finger or hand looks deformed, becomes pale or blue, feels cold or newly numb, has uncontrolled bleeding, or cannot move as expected. A tight splint with increasing pain, numbness, or colour change also needs prompt review.

Following surgery, contact the treating team for spreading redness, drainage, fever, a wound that opens, or rapidly worsening swelling and pain. Therapy exercises should be paused when they conflict with surgical precautions or produce a sudden loss of function.

Ask what structures are injured, which movements or loads are restricted, whether a splint is needed, and how progress will be measured. Clarify who to contact if swelling, sensation, wound appearance, or function changes between appointments.
