What should you know about central tfcc tear?

Short answer

A central TFCC tear is damage in the middle of the wrist's triangular fibrocartilage complex, often causing pain on the little-finger side.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Location
The TFCC lies on the little-finger side of the wrist.
Typical aggravators
Loaded gripping, wrist extension and forearm rotation can reproduce symptoms.
Assessment
Examination considers stability, bone injury, tendons, nerves and other causes of ulnar-sided pain.

Direct answer

Symptoms may be provoked by twisting a key, turning a handle, pushing up from a chair or gripping under load. Clicking can occur, but examination is needed because several wrist conditions can produce similar symptoms.

Anatomy and assessment

The TFCC is a group of cartilage and ligaments between the forearm bones and the small bones of the wrist. It helps distribute load and supports the joint near the end of the ulna. The central portion has limited blood supply, which affects how a tear may heal.

A tear can follow a fall or twisting injury, or develop with wear over time. The clinician checks the location of tenderness, grip, forearm rotation, wrist stability, sensation and circulation. X-rays can assess bone alignment or fracture, while other imaging may help evaluate soft tissues. Arthroscopy is sometimes used when the diagnosis remains uncertain or treatment is planned.

Protecting and rehabilitating the wrist

Initially, reduce forceful gripping, loaded wrist extension and repeated twisting. A clinician may recommend a splint, pain relief and a period of activity modification. Keep fingers moving unless told otherwise, and check that a brace does not cause numbness, colour change or pressure injury.

Hand therapy or physiotherapy can restore motion and build grip and forearm control as irritation settles. A cortisone injection may be considered for selected persistent symptoms, although it does not physically repair the torn tissue. If non-surgical care does not provide acceptable function, a hand surgeon may discuss arthroscopic assessment and treatment based on the tear pattern, joint stability and bone anatomy.

When wrist symptoms need prompt care

Seek urgent assessment after injury if the wrist is visibly deformed, pain is severe, or you cannot use the hand. A cold, pale or blue hand, absent feeling, rapidly increasing swelling, or inability to move the fingers requires emergency care because circulation, nerves or a fracture may be involved.

Arrange a clinical review when little-finger-side pain persists, grip remains weak, clicking is painful, or symptoms keep returning with daily tasks. Redness, warmth, fever or drainage after an injection or procedure also needs prompt medical advice.

Questions for the hand specialist

Ask whether the wrist is stable, what else could explain the pain and whether imaging would change management. Discuss which movements to avoid, how long to use a brace, when strengthening can begin, and what improvement would support continued non-surgical care. If a procedure is proposed, clarify its goal, aftercare, work restrictions and rehabilitation plan.

Questions people ask

Does every central TFCC tear need surgery?

No. Management depends on symptoms, function, stability, injury pattern and response to protection and rehabilitation. Surgery may be discussed when symptoms remain limiting despite appropriate non-surgical care.

Can a TFCC tear cause clicking?

A TFCC tear can present with painful clicking or catching, especially during rotation or loading. Clicking alone is not specific, so examination helps identify its source.

What is the purpose of a wrist splint?

A splint limits irritating movement and loading while symptoms settle. It should fit comfortably and be used according to clinical instructions to avoid unnecessary stiffness or pressure problems.
